New Sports Physical Examination Form
Medical providers conducting physicals on or after June 30, 2010 must use the form dated May 2010. The form can be found on the Oregon School Activities Association (OSAA) Website.
New Participation Fees for 2010-2011
Sport participation fees are now $225/sport. Activity participation fees are now $85/activity. Click for the complete list of fees.
Physicals
Athletic physicals will be given at Sunset High School on August 10, and at Southridge High School on August 11, from 6:00-8:30 pm. Please come to the gym. Cost is $15.00 and all proceeds go to the athletic training budget. The form is available on this web site under Fall Sports/Activities
